 インフルエンザ予防接種費用サポート申請の案内

例年通り、インフルエンザ予防接種の費用サポートの申請を11/1より開始します。対象期間は10/1-1/31に行った予防接種（国内に限る）です。　申請期間は11月-2月末までとなります。（11月にならないと申請機能がオープンしないのでご注意ください。）

ホームページにおける電子申請（インフル）ボタンから必要事項を入力し、領収書の写真添付し、申請してください。＊領収書には、1.接種者氏名（個人名）　2.接種日　3.医療機関名（領収印）　4.インフルエンザ予防接種代が分かる記述は必須となります。

申請内容に問題がなければご指定の口座(社員本人の口座に限る)へ1カ月前後でお振り込みさせていただきます。文書等による通知は行いませんのでご了承ください。

 

Flu Vaccination support

Application for Flu vaccination support has started. The target of flu vaccination support is you or your dependent family take vaccination between Oct1-Jan31 (only conducted in Japan)　　The application period is between Nov.1- Feb 28 

Please fill necessary information on the form thorough e-application button (appeared in the middle of this page) and apply.  You will confirm the payment in your applied bank account around 1month after your application.   At application, you should attach receipt from clinic which supposed to include 1) name of you/your family, 2) date of vaccination, 3)name of clinic/hospital, 4)remark as flu vaccination on the receipt.
